It's an analog device, a gender changer. I am not familiar with dreamcast. If it is a VGA connector and cable on both, adding this will change the monitor cable end from male to female.

Not sure what you mean.  There are female threaded receptacles on both sides, so the two male ends that you’re connecting to this adapter can receive the screws normally mounted on VGA ends.  There’s nothing that can be taken apart and modified; i.e., “converted.” 
 
Unless you have a space issue where you would need the connection to be shorter, having a connection with screws on one side would probably only be obtainable if you wanted extra cable length and used  a VGA cable with one female and one male end to pair with a “normal” VGA cable with two male ends. Put it this way: having three desktops with dual monitors—with each monitor having a VGA port—I needed one of these WOVTE 2-pack connector offerings.  I already had a VGA cable with a female end and a male end, so I only needed to buy one package.  I do pull out the desktops every so often for cleaning, so I needed secure cable lengths long enough to not worry about inadvertently pulling the connection(s) apart.  I did thread my screws into the female ends, although every VGA connection I’ve ever seen had a pretty tight fit even without using the screws. 
 
In short, if you’re concerned about having screws on one side, I’d recommend purchasing what I already had: a VGA cable with one female end and one male end.  Keep in mind that those might be difficult to find and probably more expensive than the two-pack of these for the current price of $5.99.  Just as well, if you don’t currently have two or more male-to-male VGA cables, then weigh the cost of whatever combination that you feel will fit your application.  Eventually—like everybody else—you’ll probably migrate away from VGA to HDMI, so purchasing the WOVTE 2-pack VGA connectors will probably be your most cost-effective option for cable extension for the time being.
